I grant permission for my child/myself, who is a jumper with the Jumping Eagles, to participate and travel with the “Jumping Eagles” competition jump rope team. I know of no medical reason why my jumper/myself should not participate as a member of the competition team or team practices. In agreeing with this waiver below, I understand and agree to absolve the Jumping Eagles Team, coaches and all sponsors of all blame for injury, misadventure, harm, loss or inconvenience suffered in any of the activities associated with their participation.

Parent: I understand the waiver from above and grant permission for my child/jumper for the 2018-19 season. Jumper over 18, I grant permission/understand the waiver for myself.

2019 Summer Competitions that are held out of state are an option
